About the position
The Transition Specialist for Indiana's Department of Correction: Division of Youth Services is responsible for overseeing transition-related practices in juvenile correctional facilities. This role involves implementing life skills curricula, evaluating transition consultations, and participating in administrative committees to support youth reentry into the community.
Responsibilities
- Facilitate the implementation of a life skills curriculum, titled Next Steps.
- Utilize the Transition Coordinator Handbook to standardize transition practices.
- Evaluate the five 'Transition Consultations' for every student under the guidance of an assigned teacher.
- Participate weekly in Administrative Release Committees (ARC) to confirm and reinforce student reentry plans.
- Conduct follow-up phone calls to released youth at 1, 3, 6, 9, and 12 months post-release.
- Represent the Indiana Department of Correction- Division of Youth Services at conferences and meetings.
- Assist in developing partnerships with Indiana's Department of Workforce Development, HIRE, and Indiana University's HOPE Mentoring.
- Help develop individualized Education Learning Plans for each youth upon entry at the facility.
- Work with state, county, and private agencies to provide re-entry services for youth transitioning back into the community.
- Assist students in obtaining necessary documentation such as social security cards, state identification, and birth certificates.
- Support students in transitioning back to public schools and provide information on employment, apprenticeship programs, and military service.
Requirements
- Valid Driver's License
- Experience in Microsoft Word, Outlook, Excel, and PowerPoint (1 year preferred)
- Strong communication skills (1 year preferred)
- Customer service experience (1 year preferred)
- Data entry experience (1 year preferred)
- Experience working with at-risk youth (1 year preferred)
