About the position

As a Transfer Center RN Coordinator with the Central Region Patient Logistics Center, you will be responsible for collecting clinical information related to transfer requests and facilitating the transfer by matching patients' clinical needs to the appropriate facility. You will coordinate the transfer of emergent, urgent, and elective patients from various referral sources, ensuring timely transfer and accurate patient information to assist with the development of an initial plan of care. You will also maintain data in Meditech and Central Logic systems, serve as a consultant for physicians and staff, and track barriers to patient intake flow to improve processes.

Responsibilities

  • Collect clinical information relative to a transfer request.
  • Facilitate the transfer by matching the patient's clinical needs to the facility with bed capacity.
  • Coordinate transfer of emergent, urgent, and elective patients to a final destination facility.
  • Ensure timely transfer and appropriate bed assignment.
  • Provide accurate patient information to assist with the development of an initial plan of care.
  • Maintain accurate and concurrent data collection in Meditech and Central Logic systems.
  • Serve as a consultant and mediator for physicians and staff.
  • Communicate with referring and receiving facilities regarding patient transfers.
  • Track and trend barriers to patient intake flow and develop action plans.
  • Demonstrate problem-solving, analytical, and critical thinking skills.

Requirements

  • Graduate of an accredited school of Nursing; BSN preferred.
  • Current licensure as a Registered Nurse in the State of Tennessee required.
  • Three years of related clinical experience in an acute care setting (ED or Critical Care preferred).
  • Must be able to work on-site at the Central Region Patient Logistics Center in Madison, TN.
  • 12 Hour Day Shift required.
  • Rotating Weekends and Holiday Shifts required.
