The Full-time Home Health Admissions Registered Nurse (RN) position involves providing care in a home health setting, focusing on admissions and start of care processes. The position requires home health experience and proficiency in OASIS documentation, ensuring that the nurse can effectively manage patient admissions and care plans.
Responsibilities
- Conduct start of care assessments for home health patients.
- Complete OASIS documentation accurately and efficiently.
- Schedule patient visits based on personal and patient needs.
- Provide high-quality nursing care in a home setting.
- Communicate effectively with patients and their families regarding care plans.
Requirements
- Must be a licensed Registered Nurse (RN) in the state.
- At least some home health experience within the last 10 years.
- Proficiency in filling out the OASIS form.
- Willingness and ability to work full-time hours.
Nice-to-haves
- Experience working with subacute Medicare patients in a home health setting.
