Responsibilities

  • Supervises day-to-day clinical and office operations.
  • Ensures services comply with professional standards, agency policies, and state and federal regulations.
  • Coordinates referrals and patient care with an interdisciplinary team.
  • Develops, implements, and updates individualized plans of care.
  • Evaluates the quality of clinical care and assesses patient needs continuously.
  • Collaborates with clinicians and physicians regarding patient transfers or discharges.
  • Manages daily KPIs, workflow, and escalation/complaint calls.
  • Guides clinicians on implementing Emergency Care Protocols.
  • Assures clinicians have required training, competency, and supervision.
  • Makes patient and personnel assignments based on demonstrated competence.
  • Provides ongoing coaching and mentoring of field clinicians.
  • Participates in quality assurance and performance improvement activities.
  • Reviews and investigates incidents, analyzes trends, and implements corrective action plans.
  • Manages accreditation/survey readiness and QA audits.
  • Activates Emergency Preparedness Planning and monitors ongoing events.

Requirements

  • Registered nurse with current licensure to practice nursing in the practicing state.
  • Possess and maintain CPR certification.
  • Previous experience in a home care setting with two years of management or supervisory experience preferred.
  • Proven ability to work within an interdisciplinary setting.
  • Must be a licensed driver with an insured automobile in good working order.
  • Prior use of Electronic Medical Records (EMR) preferred.
