About the position

The position involves directing all patient care services for the team, ensuring that services are integrated components of the interdisciplinary plan of care. Responsibilities include reviewing patient admissions, collaborating with physicians on palliative care measures, and managing team schedules to provide 24/7 service. The role also includes staff supervision, quality improvement, customer service, cost containment, and professional development.

Responsibilities

  • Directs all patient care services for the team, including volunteer and bereavement.
  • Reviews information on all pending patient admissions and new admissions to assure eligibility and appropriateness criteria are met.
  • Collaborates with team and attending physicians regarding palliative care measures for pain and symptom management.
  • Assures complete and accurate data and documentation for recertification decisions.
  • Reviews all imminent 'alive' discharges for effective discharge planning.
  • Monitors the type and level of team services provided to assure appropriateness and responsiveness to patient/family needs.
  • Manages the team schedule and coordinates services 24/7.
  • Facilitates team meetings for interdisciplinary input into the plan of care.
  • Participates in on-call rotation.
  • Interviews, selects, trains, supervises, evaluates, and dismisses team staff.
  • Assures staff competence and performance levels through field visits and evaluations.
  • Acts as a resource and mentor for staff on clinical issues and customer service.
  • Oversees staff and volunteer schedules and territory assignments.
  • Reviews and approves payroll for accurate documentation.
  • Monitors clinical records and team activities for accuracy and compliance.
  • Develops and implements performance improvement activities.
  • Ensures completion of patient/family satisfaction surveys and implements quality improvement measures.
  • Assists with surveys and requests from regulatory bodies.
  • Performs substantive chart reviews to ensure quality care delivery.
  • Participates in Outcomes Management and annual program review.
  • Performs utilization review of continuous care and inpatient levels of care.
  • Addresses problems/grievances/service failures experienced by patients/families or physicians.
  • Analyzes customer service issues to identify causes and improve performance.
  • Speaks with patients/families and attending physicians regarding potential revocation.
  • Visits LTC and Contract Bed facilities for care plan integration and customer satisfaction.
  • Participates in providing inservices to customers.
  • Verifies/approves admitting IPOC diagnosis, treatments, staffing, and supplies.
  • Monitors resource utilization for cost-effective service delivery.
  • Controls productivity, labor, and patient care costs with respect to budget.
  • Approves all bills/invoices related to patient care services.
  • Attends inservices, educational seminars, and workshops.
  • Develops and achieves professional growth goals and objectives.
  • Participates as a mentor for newly hired Team Managers.

Requirements

  • Reliable transportation with appropriate license and insurance coverage.
  • Private telephone in home.
  • Knowledge of primary medical and nursing care principles, with a working knowledge of oncologic nursing and palliative care.
  • Basic understanding of interpersonal dynamics within an interdisciplinary health care team.
  • Knowledge of the process of dying and bereavement.
  • Working knowledge of VITAS information system (Vx) and ability to utilize management reports.
  • Two years successful supervisory experience or equivalent in a health care organization.

Nice-to-haves

  • Bachelor's degree preferred.
  • Current and valid R.N. License to practice in the state where the VITAS program is located.
