About the position

The Supports Planner/Case Manager role at Beatrice Loving Heart involves providing support and care coordination for individuals within the community. The role includes assessing clients' needs, serving as the primary point of contact, and coordinating services to promote optimal health and well-being. The position offers remote training and access to necessary tools and resources.

Responsibilities

  • Evaluate the client's capacities to assist in the development of plans of service for coordination services.
  • Promote positive health behaviors to support optimal health and well-being.
  • Establish telephone contact with all clients, family members, and caregivers to assist with the coordination of services.
  • Monitor existing plans of service and provide on-site visits to individuals as needed.
  • Gather information to assist in the development of plans of service provided to clients.
  • Manage multiple priorities and tasks with flexibility to improve services rendered to clients.
  • Uphold the organization's contractual billing guidelines when documenting activities in the system.
  • Collaborate with Social Workers, Registered Nurses, and others to assist individuals with complex medical/psychological needs.
  • Educate clients on self-direction and assist with accessing resources.
  • Implement critical thinking in assessing and resolving complex client-related crises.
  • Assist clients with transitions from nursing or assisted living facilities to independence within the community.
  • Access internal and external resources for clients and collaborate with local community agencies and providers.

Requirements

  • Bachelor's degree from a four-year college or university in health/human services or related fields (psychology, sociology, counseling, physical therapy, social work, etc.).
  • Valid Maryland driver's license and reliable transportation.
  • Oral and written communication skills.
  • Ability to conduct research on a computer and other sources.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Access).
  • Ability to write routine reports and correspondence.
  • Ability to communicate effectively with individuals with varied cognitive abilities.

Nice-to-haves

  • Experience in case management or related field.
  • Familiarity with telehealth services.
  • Knowledge of community resources and support services.
