About the position

We are seeking an Adult Day Health Supervisor - PACE Program for our team to ensure that we continue to provide all patients with high quality, efficient care. Role Summary: Under the supervision of the Operations Manager, manages the daily operations of the PACE Center. Responsibilities include the planning, coordination and implementation of all operations in the PACE Center. Works in conjunction with the Social Workers to assure that concerns and support are followed up on to families, participants, and caregivers. Works in conjunction with the Therapy team (PT/OT/ST/RT) to assure that recreational activities and wellness classes are provided as scheduled. Works in conjunction with the Registered Dietician to deliver an excellent dining experience and assure that diets are provided as ordered. Works with the Triage RN to ensure that Program Aides complete assigned daily tasks and report areas of concern. At the Alternative Care Site, Supervisor serves as the Health Care Coordinator for ADHS.

Responsibilities

  • Manage the daily operations of the PACE Center.
  • Plan, coordinate, and implement all operations in the PACE Center.
  • Work with Social Workers to follow up on concerns and support for families, participants, and caregivers.
  • Collaborate with the Therapy team to ensure recreational activities and wellness classes are provided as scheduled.
  • Coordinate with the Registered Dietician to deliver an excellent dining experience and ensure diets are provided as ordered.
  • Ensure Program Aides complete assigned daily tasks and report areas of concern.

Requirements

  • Associates degree in nursing.
  • Registered Nurse license.
  • NC Driver's Licensure.
  • BLS and First Aid certification.
  • Five years of related experience with at least one year working with a frail or elderly population.

Nice-to-haves

  • Bachelor's in nursing or other healthcare related field.
  • Two years of related experience with at least one year working with a frail or elderly population.
  • Experience working effectively within an interdisciplinary team model.
  • Demonstrated effective leadership and decision-making skills with at least one year experience.
  • Previous supervisory skills including coaching and mentoring.
