About the position

The Substance Abuse LVN at Titan Recovery Center plays a vital role in supporting individuals struggling with substance use disorders. This position involves conducting assessments, developing individualized treatment plans, and collaborating with a multidisciplinary team to ensure comprehensive care throughout the recovery journey. The ideal candidate will possess a deep understanding of addiction and strong interpersonal skills to effectively guide clients and their families through the recovery process.

Responsibilities

  • Conduct assessments and evaluations to determine the needs of clients and develop individualized treatment plans.
  • Provide individual and nursing assessments focused on substance abuse recovery strategies.
  • Monitor client progress and adjust treatment plans as necessary, ensuring alignment with best practices in addiction treatment.
  • Facilitate discharge planning, ensuring clients have access to necessary resources upon completion of their treatment.
  • Collaborate with healthcare professionals, including nurses and other counselors, to coordinate care for clients in various settings such as assisted living or Level II Trauma Centers.
  • Maintain accurate and confidential client records in compliance with HIPAA regulations.
  • Educate clients and their families about substance abuse issues, coping strategies, and available support services.
  • Participate in ongoing professional development and training related to substance abuse counseling.

Requirements

  • Valid LVN license.
  • Experience working in a clinical setting, particularly with discharge planning or triage processes.
  • Strong understanding of addiction treatment methodologies and the ability to apply them effectively in practice.
  • Excellent communication skills, both verbal and written, with the ability to build rapport with clients from diverse backgrounds.
  • Familiarity with medical procedures such as venipuncture or botulinum toxin treatment is considered an asset but not mandatory.
  • Ability to lift up to 15 pounds as needed for client support or equipment handling.
  • A compassionate approach towards clients facing challenges related to substance use disorders.
