About the position

The position involves serving as a patient advocate within the FHC system, focusing on providing comprehensive support to patients and their families. The role includes receiving and reviewing referrals for behavioral health and social work, as well as providing direct services through case work and group work. The individual will assess, plan, and implement treatment plans, which encompass mental health and substance abuse counseling services, psychotherapy, and follow-up interventions based on patient progress. Evaluating clinical symptoms, developmental and personality disorders, psychosocial stressors, and global functioning is also a key responsibility. Additionally, the position requires assisting patients and their families in understanding prescribed treatments and linking them to necessary resources. Maintaining accurate case records and collaborating with community social service agencies to meet health-related needs are essential components of the role. Compliance with corporate policies and respectful interaction with patients and colleagues are expected.

Responsibilities

  • Serve as a patient advocate within the FHC system.
  • Receive and review referrals for behavioral health and social work.
  • Provide direct services to patients and families through case work and/or group work.
  • Assess, plan and implement treatment plans, including providing mental health and substance abuse counseling services, psychotherapy, and follow up with appropriate intervention for each referred patient and modify treatment plan in accordance with patient progress.
  • Evaluate clinical symptoms, developmental and personality disorders, psychosocial stressors, substance abuse/disorders, and global functioning.
  • Assist patients and their families in understanding and conforming to prescribed treatment(s).
  • Link patients and their family members to resources that provide needed services.
  • Establish and maintain accurate and complete case records, forwarding notes to healthcare providers as appropriate.
  • Maintain relationships with community social service agencies and collaborate in developing services to meet the health related needs of the community.
  • Exhibit compliance with FHC's corporate compliance policy.
  • Perform job activities in ways that are respectful, courteous and responsive while interacting with patients, visitors and co-workers.
  • Perform other duties and assume other responsibilities as appropriate or as directed.
