About the position

The Staff Registered Nurse (RN) position at Alexander Cohen Hospice House (ACHH) is a per diem night role focused on providing skilled nursing care to patients in an inpatient hospice setting. The RN will work as part of an interdisciplinary team to assess, plan, and implement care aimed at maximizing patient comfort and quality of life. This role includes responsibilities such as patient assessment, care coordination, documentation, and participation in team meetings, with a commitment to maintaining a supportive environment for patients and their families.

Responsibilities

  • Assess all patients admitted to the ACHH and communicate with the interdisciplinary team (IDT) regarding patient needs.
  • Perform reassessments based on patient acuity and provide direct patient care according to the care plan.
  • Maintain communication with the IDT regarding patient status and needs during each shift.
  • Initiate communication with physicians and community agencies to coordinate patient care.
  • Document patient care activities and changes in status accurately and timely.
  • Plan for patient discharge and coordinate with outpatient IDT for continuity of care.
  • Participate in weekly IDT meetings to update care plans and share feedback.
  • Attend inservices and workshops to promote professional growth in hospice care.
  • Coordinate physical care of patients and supervise other care staff as needed.
  • Support patients and families during the dying process by maintaining a home-like environment.
  • Assist in the orientation of new employees and act as a preceptor when assigned.
  • Participate in quality improvement processes and report quality issues as needed.
  • Follow all policies and procedures related to safety, infection control, and confidentiality.
  • Maintain a professional appearance and behavior at all times.

Requirements

  • Current license to practice as a registered nurse in the State of California.
  • At least one year of nursing care experience as a professional nurse; two years of general experience plus one year of hospice experience preferred.
  • Ability to work with an interdisciplinary team and plan patient/family care.
  • Skills in providing care in a homelike environment and addressing end-of-life issues.
  • Demonstrated ability to assess and respond to patient/family needs.
  • Skills in nursing practice including IV care, trach care, tube feedings, and emotional support for families.
  • Current CPR certification.

Nice-to-haves

  • Experience in hospice care or palliative medicine.
  • Bilingual skills for better communication with patients and families.
