About the position

The Staff Nurse is an essential member of the patient center medical home team. Responsible for managing the individualized patient care, by promoting and restoring patient's health through the nursing process; collaborating with provider and multi-disciplinary team members; provides physical and psychological support. This position will be located at various locations as assigned.

Responsibilities

  • Assess the needs of patients based on data collected through history observation, physical exam, telephone triage, and analysis or diagnostic data
  • Perform direct patient care: triage, assessment, procedures, treatment, patient education, and follow up instructions
  • Assist patients and providers in obtaining medicinal prior authorizations when required
  • Verify the patient understands the purpose of the visit
  • Provide preventative care information matching the patients age, gender, and health status
  • Maintain knowledge of the principles of the skills needed for practical nursing to provide patient care and treatment
  • Knowledge of examination, diagnostic and treatment room procedures
  • Ability to interpret, adapt, and apply guidelines and procedures
  • Basic Critical Care and Chronic Illness Management
  • Participate in training opportunities
  • Help promote office efficiency and encounter volumes
  • Understand and administer vaccines and immunizations to pediatric and/or adult patients
  • Responsible for daily clinical workflow which includes but not limited to: Referral management, telephone encounters, lab results, medication refills, lab Q&A, compliance with sample medication logs
  • Willingness to learn about AIDS Service Organizations and Community Health Centers
  • Assist in training new nurses to the organization in the standard working procedures of HopeHealth
  • All other duties assigned

Requirements

  • Diploma in Nursing; minimum requirements or Associates in Nursing
  • Licensed by the State Board of Nursing
  • BLS required
  • Two year medical office experience preferred
  • Must possess a current driver's license
  • Must have dependable, reliable transportation required to commute to other locations within the organization, as needed
  • Reliable mode of communication (mobile or home telephone) with messaging capability
  • Must possess excellent oral and written communication
  • Must have the ability to collaborate with others to improve the patient experience and address needs
  • Works with Patients, Families and Providers
  • Maintain current general medical knowledge
  • Ability to take initiative and handle various tasks simultaneously
  • Knowledge of policies, procedures, regulations, and protocols
  • Knowledge of databases (ex: EMR systems)
  • Familiar with medication names and usage
