About The Position

Requirements
- Licensed SLP
- Physical exam within 12 months prior to first patient contact
- Current TB test (within 12 months) & CPR certification
- Valid driver’s license & proof of automobile insurance
- Ability to attend 1–2 day orientation at our office
Nice To Haves
- Minimum 1 year of SLP experience preferred
- OASIS and WellSky (Kinnser) experience strongly preferred
Responsibilities
- Evaluate patients, develop individualized treatment plans, and administer therapy in the home
- Supervise COTAs and HHAs according to the treatment plan
- Chart patient progress, maintain accurate medical records, and submit notes timely
- Collaborate with families, caregivers, and interdisciplinary team
- Review and recommend adaptive equipment or home modifications as needed
- Maintain patient confidentiality per HIPAA regulations
- Coordinate total patient care and submit referrals for paramedical services
