About The Position

1to1 Rehab is seeking a part-time Speech-Language Pathologist to join their team and support an adult community caseload funded through Ontario Health at Home. Services will be delivered in clients' homes, long-term care or retirement homes, and approved community settings. This role offers the opportunity to accept referrals that align with your availability, geography, and clinical interests, build a caseload at your own pace, and provide services primarily focused on communication and swallowing in adult and older-adult populations. 1to1 Rehab is a nationally accredited, client-focused therapy service provider recognized for excellence in Speech-Language Pathology, Occupational Therapy, Physiotherapy, and Nutritional Services, supporting clients across various settings in the Greater Toronto Area. They emphasize finding the right clinician fit, offering independence, flexibility, and high-quality adult rehabilitation with strong clinical and administrative support.

Requirements

  • Master’s Degree in Speech-Language Pathology
  • Registration in good standing with CASLPO
  • Strong clinical reasoning, assessment, and intervention skills
  • Excellent communication, organization, and interpersonal abilities
  • Professional liability insurance
  • Access to a reliable vehicle and valid driver’s license
  • Clear Police Vulnerable Sector Check (within the last year)

Responsibilities

  • Completing comprehensive speech-language pathology assessments for adults, including communication (speech, language, cognitive-communication, voice, fluency) and swallowing and feeding (dysphagia).
  • Developing and implementing individualized, client-centred treatment plans aligned with best practice and Ontario Health at Home service parameters.
  • Providing functional, practical intervention strategies that support safety, independence, and participation at home.
  • Educating and coaching clients, families, and caregivers to support carryover of strategies into daily routines.
  • Monitoring progress, adjusting interventions as needed, and determining readiness for discharge or follow-up.
  • Collaborating with clients, caregivers, and interdisciplinary team members to support coordinated care.
  • Maintaining timely, accurate clinical documentation in accordance with 1to1 Rehab and Ontario Health at Home requirements.
