About The Position

Sunnyvale Gardens Post Acute is seeking a compassionate, motivated, and skilled Speech-Language Pathologist (SLP) to join our rehabilitation team. Our interdisciplinary team is committed to providing exceptional care that helps residents maximize their communication, swallowing function, cognition, and overall quality of life. The Speech-Language Pathologist evaluates and treats residents with communication, cognitive, and swallowing disorders, collaborates with physicians and interdisciplinary team members, and maintains accurate clinical documentation. The SLP also participates in care planning, staff education, and quality improvement initiatives to support optimal resident outcomes.

Requirements

  • Master's Degree in Speech-Language Pathology from an accredited program.
  • Current and active Speech-Language Pathologist license in the State of California.
  • Current Certificate of Clinical Competence (CCC-SLP) or eligibility to complete Clinical Fellowship Year (CFY).
  • Current CPR certification or ability to obtain upon hire.
  • Strong communication, organizational, and documentation skills.
  • Ability to work effectively as part of an interdisciplinary team.

Nice To Haves

  • Experience in a Skilled Nursing Facility (SNF), post-acute, rehabilitation, or long-term care setting.
  • Experience with dysphagia management, cognitive rehabilitation, and communication disorders.
  • Familiarity with electronic medical record (EMR) systems.

Responsibilities

  • Evaluate residents to determine speech therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.
  • Develop, implement, and update individualized treatment plans to improve communication, cognition, swallowing function, and overall independence.
  • Provide skilled speech-language pathology services to residents in accordance with established plans of care.
  • Monitor resident progress and modify treatment plans as appropriate to maximize outcomes.
  • Recommend and facilitate the ordering of adaptive equipment and other resources to support resident independence and safety.
  • Communicate regularly with physicians, nursing staff, interdisciplinary team members, residents, and families regarding progress, goals, and discharge planning.
  • Participate in Resident Care Conferences, Utilization Review meetings, Rehabilitation meetings, and other interdisciplinary discussions as needed.
  • Assist the nursing department with training Restorative Aides and other staff members regarding communication and swallowing-related interventions.
  • Collaborate with rehabilitation and nursing teams to ensure continuity of care.
  • Educate residents, families, and caregivers regarding treatment goals, home exercise programs, maintenance programs, aspiration precautions, swallowing safety strategies, and communication techniques.
  • Provide in-services and educational programs for facility staff as needed.
  • Serve as a clinical resource for staff regarding speech-language pathology services and best practices.
  • Complete timely, accurate, and compliant documentation in accordance with facility, governmental, Medicare, and third-party payer requirements.
  • Maintain a safe treatment environment by following infection control procedures, safety standards, and equipment maintenance protocols.
  • Contribute to quality assurance initiatives, performance improvement activities, and rehabilitation program development.
  • Maintain confidentiality of resident information in accordance with HIPAA and facility policies.
  • Supervise Speech-Language Pathologist Clinical Fellows (CFY) and students, if applicable, in accordance with state practice regulations and facility policies.
  • Provide mentorship, guidance, and clinical oversight to assigned CFY staff and students.
