Requirements
- Active California Speech-Language Pathology license
- Master’s Degree in Speech-Language Pathology from an accredited program
- Strong communication, documentation, and patient education skills
- Valid California driver’s license and reliable transportation
Nice To Haves
- CCC-SLP preferred
- Home Health or adult rehabilitation experience preferred, not required
Responsibilities
- Perform comprehensive speech, language, cognitive, and swallowing evaluations
- Develop individualized treatment plans based on patient needs and clinical goals
- Provide therapy interventions to improve communication and swallowing safety
- Educate patients and caregivers on communication strategies, swallowing precautions, and home exercises
- Collaborate closely with interdisciplinary care team members
- Monitor patient progress and modify treatment plans as needed
- Maintain timely and accurate documentation in EMR (WellSky)
