About the position

Join our passionate, employee-owned Rehab Team at Commonheart Home Health in partnership with Symbria as a Speech Language Pathologist (SLP) eager to make a real difference in the lives of older adults. We are looking for a driven team member to help provide outstanding care and support to patients in Monroe, WI, and surrounding areas. This position offers a flexible schedule, allowing you to craft your own hours with no fixed start or end times. We welcome Clinical Fellows and provide the necessary training for those just starting out.

Responsibilities

  • Set appropriate goals for your patients and creatively execute individualized treatment plans.
  • Develop and implement clinical and educational speech therapy programs.
  • Work as part of our interdisciplinary team while maintaining productivity standards and completing necessary documentation.
  • Play a key role in helping patients achieve life-changing outcomes, serving as a trusted partner to the community.

Requirements

  • Graduate of an accredited school for Speech Language Pathologists.
  • Hold a valid state license.
  • Flexible and able to work varying hours.

Nice-to-haves

  • Experience in acute care, long-term care, or rehabilitation is a bonus but not required.
