About the position

The Cambridge Health Alliance is seeking a dynamic specialty clinical pharmacist to join the Ambulatory Pharmacy Department. This role involves providing clinical specialty pharmacy services through direct patient care in ambulatory clinics or outpatient pharmacies. The pharmacist will monitor specialty drug therapy, perform medication use consults, and provide education and counseling to patients and caregivers, all while focusing on patient care, safety, and financial sustainability.

Responsibilities

  • Provide clinical specialty pharmacy services via direct patient care in ambulatory clinics or outpatient pharmacy.
  • Monitor specialty drug therapy to ensure safety and effectiveness.
  • Perform formal and informal medication use consults with appropriate documentation.
  • Educate and counsel patients and/or family members/caregivers effectively and individually.
  • Assess needs and develop or refine clinical pharmacy programs.
  • Evaluate medication use and effects on the medication use process.
  • Ensure periodic consultations with providers occur when needed.
  • Hold qualified clinicians accountable for decisions affecting consumers.

Requirements

  • Graduate of an accredited college/university with B.S. in Pharmacy or Pharm.D. degree is required.
  • Board Certification in Specialty Pharmacy is required (to be completed within two years of hire).
  • Must have a current license in good standing from the Massachusetts Board of Registration in Pharmacy or be eligible for licensure in MA.
  • Minimum of three (3) years' experience as a pharmacist, five (5) years preferred.
