About the position

We are seeking a highly motivated and experienced Outreach Health Educator to join our team on a temporary basis. Outreach Health Educators are patient health educators who are frontline public health workers with a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. The role involves developing and implementing health education programs to promote healthy lifestyles and prevent diseases. You will work closely with community organizations, schools, and healthcare providers to identify health needs and develop strategies to address them. Your ultimate goal will be to improve the health and well-being of the community we serve.

Responsibilities

  • Understand that possessing a driver license and a clean driving record/ability to be insured as a company driver is a condition of employment.
  • Available to work evenings and weekends as needed.
  • Understand that the work schedule will be assigned according to the growing season and arrivals of MSFW's.
  • Work without direct supervision, adhere to company policies, and notify the Director immediately of barriers to implementing the job as outlined.
  • Communicate and follow instructions from Outreach Care Coordinator and Community Development Director regularly regarding outreach to Migrant/Seasonal farmworkers, families, and other underserved populations in the community.
  • Participate in population-based projects that enhance retention in care, self-management, treatment adherence, and compliance with accepted health care standards.
  • Engage with community, city-wide, or statewide resources and/or professional development with other organizations such as North Carolina Farmworker Health, NC Department of Health and Human Services, North Carolina Community Health Center Association, and National Community Health Center Association.
  • Participate in assigned community-based activities, including outreach to potential new patients and connecting them to care and services, and assist with the setup and breakdown of event props.
  • Accurately complete Outreach patient assessments, PRAPARE screening tools, mileage logs, and other record keeping.
  • Interpret and translate/speak Spanish regularly for clinic operations/events.
  • Complete warm hand-offs, turning in health assessments for billing, scanning, and entry into the E.H.R to Outreach Care Coordinators or other assigned staff.
  • Initiate the patient registration process and correctly complete associated forms, gathering required documents.
  • Explain all appropriate resources for assistance including 340B, Prescription Assistance Program, ICOR, DSS, Lenoir County Transit ridership, DSS Medicaid transportation vouchers, WIC, and appointment-based care.
  • Follow protocols and utilize sound judgments to appropriately respond to requests for services, including facilitating scheduling and documenting outcomes.
  • Maintain BLS and effectively utilize CPR skills when required.
  • Regularly communicate with patients, staff, and community organizations.
  • Provide basic education, screening, and support and/or facilitate referrals to other health center or community-based resources.
  • Provide blood pressure checks, blood sugar checks, and perform other simple clinical measurements.
  • Provide transportation using a company vehicle for patients upon request, following all transportation policies.
  • Maintain a professional appearance, demeanor, and dedication to service.
  • Adhere to all federal regulations regarding HIPAA as well as KCHC employee policies.
  • Complete all other duties as assigned.

Requirements

  • High School Diploma or equivalency.
  • Valid NC driver license; acceptable driving record.
  • 1-3 years clinical/professional experience preferred; equivalent combination of education and/or experience.
  • Strong knowledge of health promotion and disease prevention principles.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
