About the position

Community Care and Hospice LLC is seeking a compassionate, bilingual (Spanish/English) Hospice Chaplain to provide spiritual support and emotional care to hospice patients and their families. This role is essential in enhancing comfort, promoting quality of life, and supporting dignified end-of-life care, working closely with an interdisciplinary care team to meet the spiritual needs of a diverse patient population.

Responsibilities

• Provide spiritual care and counseling to patients and families in both English and Spanish, respecting each individual's cultural and religious preferences.
• Visit patients regularly and offer spiritual guidance, prayer, or religious rituals when requested.
• Participate in the hospice care team to ensure comprehensive support for patients, addressing both physical and emotional/spiritual needs.
• Assist in the creation and delivery of individualized care plans that incorporate spiritual care.
• Offer bereavement support services to families, both during the hospice care and after the loss of a loved one.
• Facilitate memorial services or spiritual ceremonies as requested by families.
• Maintain appropriate documentation of spiritual care provided and participate in team meetings.

Requirements

• Fluency in both Spanish and English, with excellent communication skills in both languages.
• Ordained or credentialed by a recognized religious organization or faith group.
• Experience in hospice or end-of-life care is preferred, but a strong background in pastoral care will be considered.
• Ability to work compassionately with individuals from diverse faith backgrounds, providing non-judgmental support.
• Emotional resilience and a strong commitment to providing support during challenging times.

Nice-to-haves

• Experience in the Ventura or Greater Los Angeles area, with familiarity with local resources and communities.
• Prior experience offering bereavement support or grief counseling.
