About The Position

We are seeking a Social Worker for our busy Medicine units. The Social Worker plays a critical role in the process of developing comprehensive discharge plans, beginning on the day of admission. The Social Worker aids in the care, development, education and support of all the patients and provides support to the families. Assist in enhancing their capabilities of coping with illness and daily functioning. The goal is to provide the highest quality medical care and transition of care for the patient. This is a Day shift, Monday - Friday position with rotating weekend/holiday coverage.

Requirements

• Master’s Degree in Social Work (MSW)
• New York State License as a Licensed Masters Social Worker (LMSW)
• New Hires pending a NYS Licensed Master Social Worker (LMSW), must have Limited Permit (must supply exam date prior to start of employment); the LMSW must be obtained within 1 year of hire

Nice To Haves

• At least one year of inpatient Medical/Surgical experience
• Discharge Planning experience
