About the position

CareWell Services Southwest is seeking a dedicated full-time Social Worker (LLMSW, LMSW, LLBSW, or LBSW) to join our Supports Coordination Team. This role is pivotal in promoting health, independence, and choice for seniors, adults with disabilities, and their caregivers. As an Area Agency on Aging, we focus on identifying the needs of our participants and developing tailored plans that enable them to maintain their independence with dignity through person-centered services. In this role, you will perform assessments and coordinate activities related to the social, emotional, and financial needs of participants. You will evaluate care plans to ensure they meet individualized goals and needs. Your responsibilities will include conducting functional assessments of participants' social, financial, environmental, health, and emotional needs, facilitating the development of person-centered service plans, providing advocacy, and arranging care as specified in these plans. You will maintain electronic files for participants and utilize a tracking system for those receiving services. Collaboration with staff to assess and manage individual case details is essential, as is maintaining state standards of quality care and service provisions. You will also provide information and assist participants in connecting to community services and resources for natural support, ensuring that all documentation, including assessments and progress notes, is completed promptly and in compliance with agency and state procedures.

Responsibilities

  • Conduct functional assessments of the participant's social, financial, environmental, health, and emotional needs.
  • Facilitate and coordinate the development of a person-centered service plan of care in conjunction with the participant and other identified supports.
  • Provide advocacy and arrange for care as specified in the participant's person-centered service plan of care.
  • Maintain participant electronic files while utilizing a tracking system for participants receiving services.
  • Consult with staff for assessing and managing individual case details.
  • Maintain state standards of quality care and service provisions for participants.
  • Provide information and assist participants with connecting to community services and resources for natural support.
  • Document all assessments, person-centered service plans of care, and progress notes promptly while ensuring all Agency and Michigan Department of Health and Human Services (MDHHS) procedures are met.

Requirements

  • Must have current Limited or Full State of Michigan Social Work license.
  • Experience in developing care plans is preferred.
  • Bilingual skills are a plus.
