About the position

The Social Worker position within the RRTP program focuses on providing treatment coordination and therapeutic interventions for Resident Veterans facing various mental, addiction, behavioral, physical, psychosocial, and environmental challenges. The role aims to maximize rehabilitation and treatment potential, assisting Veterans in achieving stability in emotional, economic, and housing aspects before reintegrating into their communities.

Responsibilities

  • Independently implement the recovery model of treatment and carry a caseload.
  • Develop treatment plans and mutually establish treatment goals with Veterans based on initial assessments.
  • Provide treatment coordination services from admission until discharge, working closely with the treatment team.
  • Conduct daily group treatment coordination meetings for assigned caseload and meet individually with Veterans bi-weekly.
  • Facilitate a minimum of 5 group hours per week and additional individual sessions as needed.
  • Coordinate community-based services and provide information and referrals to other VA and community programs.
  • Identify high-risk patients and provide crisis intervention services, including family conferences.
  • Establish and maintain effective therapeutic relationships with Veterans and their families.
  • Provide consultation services to staff regarding the psychosocial needs of Veterans.
  • Evaluate own practice through peer review and case conferences.

Requirements

  • Experience in treatment coordination and discharge planning within the VA system.
  • Ability to work independently with a caseload of Veterans.
  • Knowledge of medical and mental health diagnoses, disabilities, and treatment procedures.
  • Experience in individual, group, and family counseling and therapy skills.
  • Effective communication skills, both orally and in writing.

  • Familiarity with Veterans' benefits and services related to special programs.
