About The Position

Position Overview: The Pediatric Intervention Associate supports the Collaborative Care for Youth Innovation grant and works within the Collaborative Care Model (CoCM) and Primary Care Behavioral Health Model (PCBH) . Under the supervision of Master’s-level Interventionists and Licensed Clinicians, the Associate supports pediatric and young adult behavioral health services in primary care settings, as well as access to care for children and families and helps address behavioral health needs among pediatric and vulnerable populations. Assists with behavioral health screenings, intervention activities, care coordination, documentation, and referral support for patients’ ages 0–26. As a member of the collaborative care team, the Associate works closely with primary care providers, medical staff, and behavioral health professionals to support integrated, family-centered care. This position is based in Community Memorial Healthcare’s outpatient primary care clinics and does not include hospital or inpatient responsibilities. This is a grant-funded position with a three-year term and possibility of extension.

Requirements

  • Bachelor’s Degree in Social Work, Counseling, Psychology, or Marriage and Family Therapy
  • One (1) year of experience working with children, adolescents and/or adults in a healthcare, behavioral, mental health, or social services setting
  • Current and ongoing Motor Vehicle Report (MVR) considered insurable under CMH insurance coverage
  • Current California Driver’s License

Nice To Haves

  • Two (2) years of experience working with children and youth, ages 0-18
  • Bilingual in Spanish and English
  • Basic knowledge of pharmacotherapy
  • Strong organizational, communication, and documentation skills
  • Proficiency in electronic health records and data tracking systems
  • Training in evidence-based interventions (COCM, PCBH, CBT, MI, PST, or trauma-focused models)
  • Ability to manage multiple priorities and maintain professional composure in a fast-paced environment
  • Demonstrated ability to work collaboratively with multidisciplinary teams in primary care settings
