About The Position

Under general supervision, supports family engagement and retention through service navigation, follow-up, and coordination. Maintains regular contact with enrolled families based on service intensity and communicates emerging needs to the POSC Coordinator to inform service plan updates. Services are provided in the client’s health department, home or school. Travel is required. This position is a Grant funded position.

Requirements

• Bachelor’s degree in a related field from an accredited college or university.
• Two (2) years of experience in social service delivery.
