About the position

The Social Worker, MSW position is a full-time remote role focused on providing essential social work services to patients and families within a healthcare organization. The role involves conducting psychosocial assessments, crisis intervention, and case management to support members in navigating their health plan benefits and accessing necessary resources. The Social Worker will play a critical role in improving the quality of life for members by addressing their unique needs, particularly in the context of managed care and health plans.

Responsibilities

  • Manage a caseload focused on social determinants of health, behavioral, and psychosocial needs.
  • Conduct telephonic and/or in-person assessments to evaluate needs like housing, food security, and transportation.
  • Coordinate with the health plan, primary care providers, and healthcare teams for member support.
  • Refer members to community resources, Long Term Support Services (LTSS), and behavioral health services.
  • Help members understand and access their health plan benefits.
  • Offer information on Advanced Directives and end-of-life planning as needed.
  • Provide counseling and crisis support for psychosocial issues.
  • Report suspected abuse/neglect cases to the appropriate teams.
  • Maintain accurate documentation in Prospect's care management system.
  • Track and update a resource library relevant to the needs of our member population.
  • Deliver in-service training to staff on social service issues and community resources.
  • Participate in social work projects and initiatives.
  • Meet departmental productivity standards and complete required trainings.

Requirements

  • Master's degree in Social Work (MSW or LCSW) is required.
  • Minimum of 2 years of experience in ambulatory case management, preferably with a health plan or medical group.
  • Experience working with diverse populations, including underserved communities.

Nice-to-haves

  • Bilingual skills are preferred.
  • Experience working with seniors or people with disabilities is preferred.
  • Familiarity with HMO, Medi-Cal, or health services is preferred.
