About the position

The Social Worker at AdventHealth Hospice Care plays a crucial role in managing the emotional and social service needs of patients and families receiving hospice services. This position involves coordinating social service interventions, conducting needs assessments, and providing supportive counseling to enhance the well-being of patients and their families during the dying process.

Responsibilities

• Evaluate the psychosocial needs of each patient and family care unit.
• Develop and implement the psychosocial plan of care in response to the needs of the patients and families.
• Travel to patient residences and perform initial assessments.
• Identify and assess the psychosocial needs of the patient and family, including caregiving relationships and dynamics.
• Assist patients and families with obtaining financial resources.
• Provide services as determined by the interdisciplinary team and report back to the registered nurse regarding assessments and needs for other organizational resources.
• Communicate with caregivers and make referrals for intra-organizational services and community resources such as Medicaid, Salvation Army, and food banks.
• Provide education and assistance in completing Advance Directives.

Requirements

• Master's degree in Social Work is required.
• A minimum of 2 years' experience in the social work field is required.
• A clear/active Social Work license for the state of Florida or license-eligible under Florida law is required.
• Maintain a current, valid Florida Driver's License in good standing with a safe driving record and a reliable vehicle.
• Maintain valid personal automobile insurance.
• Basic Life Support Certification.

Nice-to-haves

• Healthcare/hospice experience is preferred.
• A clear/active Social Work license for the state of Florida.
