About the position

As a Full Time Hospice Clinical Social Worker at AccentCare, you will play a vital role in providing compassionate care to patients and their families in the Boston area, specifically covering hospitals such as Beth Israel and Boston Medical Center. This position offers a unique opportunity to impact the lives of patients during their health journey while creating meaningful memories. The role includes a competitive salary range of $65,000 to $87,000 per year, with a schedule of Monday to Friday from 8 am to 5 pm, plus on-call responsibilities. A sign-on bonus of $10,000 is also available for new hires.

Responsibilities

• Provide emotional support and counseling to patients and their families.
• Assist patients in navigating healthcare systems and accessing resources.
• Collaborate with healthcare teams to develop and implement care plans.
• Conduct assessments to determine patients' needs and preferences.
• Educate patients and families about hospice services and options.

Requirements

• Master's Degree from an accredited school of Social Work or related field.
• Valid license/registration or permit to practice in the state(s) of agency operation.
• Medicare/Medicaid/Insurance specialty preferred.
• Licensed to practice as a clinical social worker in the state of agency operation if required.
• Must be a licensed driver who can travel to all business locations.
