The Ambulatory Care Manager role at Corewell Health West focuses on providing comprehensive care management for patients with behavioral health concerns and chronic medical conditions. This position involves collaborating with primary care providers to develop treatment plans, offering support to patients and their families, and ensuring continuity of care. The role also emphasizes the importance of addressing Social Determinants of Health (SDoH) to improve overall health outcomes in the community.
Responsibilities
- Plans and prioritizes care for individuals and populations, focusing on strategies that promote optimal health.
- Provides peer support and role modeling related to life skills and health/wellness during home visits.
- Demonstrates expertise in patient care and manages a caseload of clients of varying complexity.
- Assesses internal referrals and provides individual, short-term therapy or care management services, monitoring patient progress.
- Develops plans of care and makes recommendations to primary care providers and specialists regarding mental health treatment strategies.
- Assesses and provides follow-up for the Systematic Case Review process, consulting with psychiatrists and assisting in psychotropic medication management.
- Responds to patients' urgent mental health concerns, including suicide risk assessments, and assists patients in accessing appropriate care.
Requirements
- Master Degree in Social Work (MSW) - Required
- 2 years of relevant experience in a related field - Required
- Licensed Master Social Worker (LMSW) in the State of Michigan upon hire - Required
