About the position
The Social Worker position within the Homeless-PACT (H-PACT) program focuses on providing case management services to veterans who are homeless or at risk of homelessness. The role involves assessing needs, developing care plans, and coordinating with community resources to support veterans in maintaining housing stability. The position requires strong interpersonal skills and the ability to work independently in a dynamic environment.
Responsibilities
- Provide case management services to veterans enrolled in the H-PACT program.
- Triages walk-ins at the H-PACT clinic and completes needs assessments.
- Independently develops appropriate plans of care based on assessments.
- Plans and maintains relationships with community resources for unhoused individuals.
- Makes referrals to other agencies and community resources to resolve issues.
Requirements
- Must be a U.S. Citizen.
- Master's degree in social work from an accredited school.
- Licensed or certified to independently practice social work at the master's level.
- Proficient in written and spoken English.
