About The Position

Medical Hill Healthcare Center is seeking a compassionate, organized, and dedicated Social Services Director to lead our social services department. In this important leadership role, you will help ensure that residents receive the emotional support, advocacy, and care coordination they need to maintain dignity, independence, and quality of life. We are looking for a professional who is passionate about resident-centered care and thrives in a collaborative healthcare environment. Position Summary The Social Services Director is responsible for planning, organizing, and directing the overall operation of the social services department. This role works closely with residents, families, interdisciplinary teams, and community resources to support residents’ psychosocial well-being and successful transitions of care.

Requirements

  • Bachelor’s Degree in Social Work, Human Services, or related field required
  • Strong communication, leadership, and organizational skills
  • Ability to work effectively with residents, families, and interdisciplinary teams

Nice To Haves

  • Previous experience in healthcare, skilled nursing, or long-term care preferred
  • Knowledge of discharge planning, resident rights, and community resources preferred
  • Compassionate and resident-centered approach to care

Responsibilities

  • Develop, implement, and oversee social services programs that promote resident well-being, independence, and dignity
  • Provide emotional support, counseling, and advocacy for residents and families
  • Coordinate discharge planning and facilitate smooth transitions of care
  • Collaborate with physicians, nursing staff, therapy teams, and outside agencies to ensure comprehensive resident support
  • Arrange ancillary services, transportation, and outpatient appointments as needed
  • Assist residents and families with community resources and long-term care planning
  • Oversee resident personal property management, including inventories and lost-item documentation
  • Maintain accurate documentation in compliance with facility policies and regulatory standards
  • Serve as a liaison between residents, families, staff, and external agencies
  • Participate in care plan meetings and interdisciplinary team discussions
