About the position

The Social Services Assistant plays a vital role in supporting individuals and families in navigating social services. This position involves providing guidance and facilitating access to essential resources, requiring a strong commitment to helping others and the ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Make clear assessments and develop appropriate treatment plans based on needs and priorities.
• Provide direct social work services and counseling to enhance psychosocial functioning, spiritual, cultural, emotional, financial, and physical well-being.
• Initiate the interview process with residents/families within 24 hours of admission to determine their needs and formulate a written assessment within 2 days.
• Develop a plan of intervention based on assessments, addressing residents' physical and psychological problems in coordination with other healthcare disciplines.
• Assist residents/families through education and conferences to understand and follow medical recommendations.
• Perform interdisciplinary discharge planning and review for each resident within 48 hours of admission, with ongoing evaluation and documentation.
• Conduct regular room visits to residents and maintain contact with families by phone or mail.
• Maintain resident dignity, quality of life, confidentiality, and serve as an advocate for residents.

Requirements

• Experience in discharge planning.
• Ability to communicate effectively with residents and families.
• Strong problem-solving skills to address adjustment concerns or conflicts.
• Knowledge of clinical interventions with individuals and groups.
