Responsibilities
- Serve as an advocate for residents and their families
- Work closely with residents and their loved ones to ensure their physical, emotional, and social needs are met
- Document residents' charts
- Conduct assessments and coordinate various aspects of care
- Promote a positive and supportive environment for residents
- Discharge planning
- Collaboration with multidisciplinary teams
Requirements
- Strengths in the area of Social Services and Case Management
- Experience in long term and memory care of at least 1 year
- Ability to work with a multidisciplinary team
- Excellent written and verbal communication skills
- Knowledge of HMO, Medicare, and Insurance preferred
- Knowledge of community resources
- Ensure compliance with all HIPAA regulations
- Passionate about social services and case management
