About the position
The Service Coordinator at North Bay Regional Center is responsible for managing the delivery of services to clients with developmental disabilities, particularly focusing on young children aged 3-6. This role involves implementing service plans, conducting assessments, and ensuring that clients receive appropriate resources and support. The position requires a strong commitment to advocacy and care coordination, working closely with families and community organizations to empower clients to achieve their goals and live independently.
Responsibilities
- Manage the delivery of services to regional center clients, including those with intensive physical or emotional needs.
- Implement clients' service plans and monitor vendor services to ensure compliance with purchase authorizations.
- Conduct annual reassessments of clients' developmental status in collaboration with the interdisciplinary team.
- Assist in the Lanterman eligibility determination process for children under 'Provisional Eligibility'.
- Evaluate the effectiveness of clients' Individual Program Plans and make necessary adjustments.
- Occasionally act in a supervisory or lead capacity, such as Officer of the Day or Acting Supervisor.
Requirements
- Master's degree in a field related to developmental disabilities, or a Bachelor's degree with three years of relevant experience.
- Fluency in Spanish and English is required, with bilingual skills preferred.
- Possession of a valid California driver's license and minimum legal automobile liability insurance.
