About the position

The Service Coordinator for Early Intervention at RISE Early Intervention plays a crucial role in supporting families of children aged birth to three years with disabilities or special developmental needs. This position is part of a multidisciplinary team that focuses on enhancing the child's growth and development in their natural environment, ensuring that services are relevant and meaningful to the family's daily routines.

Responsibilities

  • Provide supports and services in the context of the family's typical routines and activities.
  • Process referrals and schedule initial meetings with families.
  • Conduct screenings using standardized protocols and tools approved by AzEIP.
  • Ensure timely delivery of services and proper maintenance of child files and documentation.
  • Coordinate and schedule assessments, IFSP meetings, IEP meetings, and transition planning.
  • Demonstrate knowledge in AzEIP Policies, Child Development, and Family Professional Partnerships.

Requirements

  • Bachelor's Degree in Early Childhood, Child Development, Nursing, Psychology, Social Work, Special Education, or a closely related field.
  • Pediatric experience is a plus.
  • Demonstrative communication and listening skills, both written and verbal.
  • Ability to travel for meetings, training, and appointments.
  • Successful completion of a background clearance and obtaining Level One Fingerprint Card Clearance.

Nice-to-haves

  • Bilingual English/Spanish is a plus.
