About the position

The Housing Liaison will become the Summit ElderCare subject matter expert for low income, and public, senior housing. In this role, the Housing Liaison will work closely with housing leadership and resident support staff to identify opportunities to introduce the Summit ElderCare option to residents. This role will collaborate closely with housing providers, and ancillary resources that support residents such as ASAPs, management companies, etc. This role will increase referrals from housing providers.

Responsibilities

  • Increase Summit ElderCare visibility across housing partners
  • Ensure a high level of engagement with key housing resources across priority service areas (including account management, program coordination, outreach & education, and engagement with appropriate senior services providers)
  • Build relationships with key roles within housing communities
  • Manage scheduled housing programming
  • Support program growth by driving a measurable increase in referrals from housing partners
  • Collaborate closely with Account Executives to ensure optimal referral management
  • Identify and collaborate with new housing development opportunities
  • Support creation of additional SE 'supportive housing' opportunities
  • Support program growth by identifying new senior housing collaboration opportunities
  • Timely and accurately documents activities in the department's CRM application
  • Consistently meets activity deliverables, tracks them in the CRM and Outlook calendar
  • Regularly reviews effectiveness of efforts against goals
  • Provides market intelligence to the Director, PACE Growth & Engagement so that adjustments can be made to tactical plans
  • Manages own professional development by seeking advice, training, and coaching from colleagues and leader
  • Acts as a resource for colleagues as required and in support of special projects that tie to program goals
  • Completes all assigned Fallon Health educational requirements in a timely manner
  • Follows outreach guidelines and regulations provided by CMS and EOHHS
  • Seeks partnership and assistance on any questions pertaining to compliance or privacy regulations

Requirements

  • BS or BA degree and/or comparable experience
  • Reliable transportation required for marketing activities and site visits
  • 2 or more years' experience with senior housing services
  • Experience with managing accounts, customer service, networking, and relationship building
  • Proficiency with Microsoft Office Suite and experience using technology-based applications and tools

Nice-to-haves

  • Knowledge of Medicare, Medicaid and/or other insurance products is desirable
  • Knowledge of the PACE program is desirable
  • Understanding of the senior services market is desirable
