About The Position

Haven Hospice is seeking a passionate, dedicated Triage RN to join their team. This role involves providing calm, confident support to patients and families during critical moments, acting as a lifeline after hours. The Triage RN will guide, comfort, and coordinate care when it matters most, making a lasting impact. This is an onsite position based in Lake City, covering all of Haven Hospice's service area.

Requirements

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• 1 year hospice nursing experience required.
• Nursing Compact RN license required.

Nice To Haves

• 3+ years hospice nursing experience preferred.
• Experience in hospice or similar setting preferred.
• Prior experience with triage, preferred.

Responsibilities

• Answer hospice calls during nights, weekends, holidays, and emergencies.
• Help schedule nurse visits, handle emergency patient admissions, and dispatch on-call staff.
• Talk with patients and caregivers to assess needs and provide support over the phone.
• Communicate with doctors about patient updates and get new care orders if needed.
• Give instructions and guidance based on each patient’s care plan.
• Coordinate after-hours needs such as equipment, supplies, and medications.
• Work with facility staff to support their patients during off-hours.
• Document all calls, care updates, and decisions clearly and accurately.
• Ensure all care and procedures follow state and federal rules.
