About the position

The position involves working from a centralized communication center that serves as a multi-line hub for triaging and providing health information to patients. The primary responsibility is to prioritize incoming calls based on the clinical acuity of the patients. This role requires assessing the needs of patients, developing care plans based on phone triage assessments, and directing patients to the appropriate healthcare provider or facility when necessary. The position is not remote or work-from-home, emphasizing the importance of being physically present in the communication center. In addition to triaging calls, the role includes providing patient education regarding disease management and medications, offering anticipatory guidance and counseling. Healthcare recommendations are made based on the assessment of patient needs, utilizing approved protocols. The nurse will also process prescription refills, assess patients by obtaining focused health histories, and document all call inquiries accurately. Communication with the Director and/or physician regarding complex triage calls is essential, as is taking verbal orders when required. The nurse will assist with statistical data collection and provide healthcare information to patients through telecommunications media, employing the nursing process. Collaboration with other departments and pharmacies regarding patient needs is also a critical aspect of the role. The nurse will be responsible for ordering labs and testing, as well as scheduling appointments with the office as needed. Adherence to all organizational and departmental policies and procedures is mandatory.

Responsibilities

Requirements

  • Certificate in Nursing required.
  • 3 years of experience as a Staff RN required.
  • Registered Nurse (RN) licensure in the Commonwealth of Pennsylvania required.
  • Current licensure either in the state where the facility is located or a multistate license issued by a participating NLC state.
