About the position

The primary function of this position is to deliver patient care utilizing the nursing process through data collection and assessment, identifying and determining the priority of patient's problems/needs, and evaluating the process and outcome of nursing care. The role also involves coordinating, providing, and directing patient teaching activities and coordinating the care provided by health team members.

Responsibilities

• Conduct initial and ongoing patient assessment in collaboration with the interdisciplinary care team.
• Analyze assessment data and create a plan of care.
• Implement treatment and evaluate treatment effectiveness.
• Administer medications safely in accordance with State of Practice and organization policies.
• Serve as point of contact for patients' care coordination throughout hospital departments.
• Act as a patient safety advocate by participating in ongoing quality improvement in the department.
• Incorporate the professional practice model into care delivery.
