About the position
The Travel RN-Case Manager position involves providing nursing care and case management services to patients while traveling to various locations in California. This role is designed for registered nurses who are looking for a temporary assignment of 13 weeks, working from 8:00 AM to 4:30 PM. The position offers a unique opportunity to explore California while fulfilling professional responsibilities in a rewarding travel nursing role.
Responsibilities
- Provide case management services to patients in various healthcare settings.
- Coordinate patient care and ensure effective communication among healthcare providers.
- Assess patient needs and develop individualized care plans.
- Monitor patient progress and adjust care plans as necessary.
- Document patient interactions and maintain accurate medical records.
Requirements
- Current RN license in California.
- Previous experience in case management or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
Nice-to-haves
- Experience in travel nursing.
- Certification in case management (CCM) or similar.
