About the position
The Hospice House Registered Nurse (RN) position at Granite VNA, Inc. involves delivering age-appropriate care to patients admitted to the Hospice House during evening shifts. The RN will utilize standard nursing best practices to assess, plan, implement, and evaluate patient care within the scope defined by the NH Board of Nursing. This role requires effective interaction with patients, caregivers, and healthcare team members while maintaining professional nursing standards.
Responsibilities
- Deliver age-appropriate care to patients in the Hospice House.
- Explain the Hospice program philosophy, including services and limitations.
- Develop and implement a plan of care (POC) in compliance with referral source orders and treatment measures.
- Document ongoing assessments, treatments, and discharge plans accurately.
- Engage patients and caregivers in meaningful education as indicated in the care plan.
- Communicate with physicians and healthcare practitioners regarding the POC.
- Coordinate the patient's POC with the Interdisciplinary Team and other health professionals.
- Supervise Licensed Practical Nurses and Licensed Nursing Assistants assigned to patients.
- Coach, mentor, and precept new nursing employees and students.
Requirements
- Current RN License in New Hampshire or compact state license through the Nurse Licensure Compact (NLC).
- Minimum of two years of acute care nursing experience.
- Three to four years of recent general medical/surgical or skilled/acute care nursing experience.
