About the position
The Registered Nurse (RN) in Home and Community Based Services is responsible for providing and directing nursing and personal care to patients in their homes. This role emphasizes flexibility, work-life balance, and the opportunity for career growth while fostering trusted relationships with patients and their families. The RN will coordinate care with other healthcare professionals and ensure high-quality clinical services are delivered in compliance with applicable laws and agency policies.
Responsibilities
- Provide and direct nursing and personal care to patients in their homes as prescribed by the physician.
- Coordinate the plan of care with other healthcare professionals involved in patient care.
- Achieve and maintain continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, and case managers.
- Complete clinical nursing assessments in accordance with federal and/or state program requirements.
- Ensure patient/client eligibility and medical necessity for services as defined by payer source and agency policy.
- Develop and revise individualized plans of care according to federal and/or state program requirements.
- Ensure plan of care frequency and duration meets the needs of the patient and initiate revisions as needed with physician approval.
Requirements
- A minimum of one year experience as a Registered Nurse (RN).
- Current RN licensure in the state of practice.
- Current CPR certification required.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
