About the position
BAYADA Home Health Care is seeking a Registered Nurse (RN) to fill the position of Home Health Nurse Manager for our Wilmington, MA Medicare Certified home health office. This role involves leading and managing a team to provide exceptional care to adult and geriatric clients, ensuring high-quality services and effective communication among team members.
Responsibilities
- Lead, inspire and motivate others to provide exceptional care.
- Work closely with the Rehab Manager, Director, and DCO to ensure cohesive management of all field staff.
- Review all clinical documentation to ensure consistent, accurate, high-quality services.
- Assign the most appropriate clinicians to each client based on their needs.
- Develop, implement, and update the client's individualized plan of care based on continual assessment.
- Coordinate client care, referrals, and act as internal case manager and client advocate.
- Communicate and collaborate with team members regarding client status and assignments.
- Participate in the hiring of field staff and interview process.
- Represent BAYADA in community and marketing activities.
Requirements
- RN License
- 2 or more years of experience as an RN in Home Health
- 2 or more years of experience in a leadership/supervisory role
- Excellent organizational skills
- Strong communication skills
- Knowledge of home health policies and procedures
