About the position

The EmPATH Registered Nurse is a skilled professional responsible for delivering high-quality, individualized patient care in a mental health setting. This role involves organizing, planning, directing, and coordinating patient care based on the Watson Model of Care, with a focus on meeting the special needs of patients. The nurse will work closely with physicians and other healthcare providers to assess and manage patients' mental health conditions, ensuring safety and effective treatment planning.

Responsibilities

  • Provides nursing care to mental health clients with unstable conditions.
  • Evaluates patients for suicidal or homicidal tendencies in collaboration with physicians.
  • Oversees direct patient care and performs patient assessments.
  • Conducts Behavioral Health assessments and ensures safety at all times.
  • Provides treatment and discharge planning by interfacing with relevant resources.
  • Utilizes nursing process as a problem-solving model to meet patient needs.
  • Recognizes subtle changes in clinical situations and alters care plans accordingly.
  • Prioritizes and organizes patient care, ensuring follow-through with planned care.
  • Develops and maintains therapeutic nurse/patient relationships.
  • Conducts hourly rounding on patients and communicates using SBAR during hand-offs.
  • Documents patient care including assessment, implementation, and evaluation.
  • Meets the learning needs of patients and families to achieve positive outcomes.
  • Communicates changes in patient condition to physicians and team members promptly.
  • Plans, supervises, instructs, and evaluates ancillary nursing personnel.
  • Maintains a safe and clean environment compliant with regulatory standards.
  • Participates in the implementation of the unit-specific Quality and Performance Improvement Plan.

Requirements

  • Associate's degree in nursing or higher level of nursing degree required.
  • Minimum 2 years of experience as an RN in acute adult outpatient or inpatient psychiatric unit.
  • Current California Nursing license.
  • Basic Life Support (BLS) certification required.
  • Knowledge of psychiatric and medical-surgical illnesses and treatment.

Nice-to-haves

  • BSN preferred.
  • Bilingual skills preferred (Spanish).
  • Basic computer skills.
  • Electronic Medical Record experience preferred.
