About The Position

Navitas Healthcare, LLC is seeking an RN Case Manager for an exciting Travel or Local job in Fort Walton Beach, FL. This role involves guiding patients and families through rehabilitation and care program orientation, educating them on treatment plans, insurance benefits, discharge criteria, and patient rights. The position requires developing and implementing individualized patient-centered care plans, completing psychosocial assessments and discharge planning evaluations, and coordinating family conferences and interdisciplinary treatment discussions. The RN Case Manager will collaborate with physicians, nurses, and care teams, advocate for safe discharge planning and continuity of care, and maintain accurate and timely clinical documentation to promote compassionate and high-quality patient care.

Requirements

Active Registered Nurse (RN) license required
Associate Degree or Nursing Certificate required
Minimum 2 years acute care experience within last 3 years required
Strong communication, critical thinking, and patient advocacy skills required

Nice To Haves

Preferred backgrounds: ICU, CCU, Med-Surg, LTAC, or Case Management
