The RN Case Manager at United Memorial Medical Center serves as a patient advocate, guiding patients through their healthcare journey while ensuring high standards of care. The position also includes conducting quality assurance and quality improvement screenings to support the medical staff's programs, and formulating discharge plans in collaboration with patients and multidisciplinary teams.

Responsibilities

• Serve as a patient advocate, guiding patients through the healthcare process.
• Monitor admissions to establish medical necessity and appropriate levels of care.
• Facilitate transitions to alternate post-acute care settings when appropriate.
• Conduct QA/QI screenings to support the medical staff's QA/QI program.
• Formulate appropriate discharge plans in conjunction with patients and the multidisciplinary team.

Requirements

• Current Registered Nurse licensure in the State of New York required.
• Annual BLS certification required.
• ASN with a minimum of 3-5 years of Care Management experience.
• Bachelor of Science in Nursing or related healthcare field preferred.
• At least three years of clinical experience in direct patient care.
