About the position

The RN Case Manager (RNCM) plays a crucial role in providing skilled nursing assessment, care planning, and interventions for hospice patients and their families. Working under the direction of the Patient Care Manager, the RNCM coordinates communication among the interdisciplinary team, ensuring that patient care is aligned with hospice policies and procedures to enhance the quality of life for patients in their final stages of life.

Responsibilities

  • Admissions of new patients, including assessment, planning, implementation, and evaluation phases of the nursing process.
  • Perform scheduled home visits to assess needs and teach caregivers about pain management and patient care skills.
  • Maintain regular communication with the interdisciplinary team regarding patient status and any issues.
  • Initiate communication with attending physicians and community agencies to coordinate care.
  • Maintain up-to-date patient records and ensure documentation is completed timely.
  • Attend regularly scheduled Interdisciplinary Team Meetings to review patient needs and share feedback.
  • Coordinate physical care of the patient, including supervision of Home Health Aides and Licensed Vocational Nurses.
  • Ensure continuity of care for patients in all settings, coordinating transfers and care plans as needed.
  • Provide 24/7 on-call coverage for patients and families as part of a rotation.
  • Attend death events and provide support to families during bereavement.
  • Participate in the education of students and community healthcare professionals.
  • Engage in quality improvement processes to enhance service delivery.
  • Attend inservices and workshops for professional growth and understanding of hospice care.
  • Follow all Community Hospice policies and procedures, ensuring compliance with safety and confidentiality standards.
  • Maintain a professional appearance and behavior at all times.

Requirements

  • Graduation from an accredited school of nursing; BSN preferred.
  • At least one year of recent experience as a professional nurse or a BSN from an accredited program.
  • Current license to practice as a registered nurse in California.
  • Valid California driver's license and current automobile insurance.
  • Possession of current CPR certification.

Nice-to-haves

  • CHPN certification preferred.
