About the position
The Registered Nurse Case Manager (RNCM) at Altus Hospice is responsible for planning, organizing, and directing hospice care for patients and their families in the Marion area. This role emphasizes compassionate care, effective communication, and collaboration with interdisciplinary teams to ensure high-quality end-of-life care. The RNCM will utilize their nursing skills to assess, implement, and evaluate patient care while maintaining professional standards.
Responsibilities
- Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
- Partner closely with the Clinical Director to assess the eligibility and suitability of clients for hospice services.
- Provide direct care to patients, ensuring their comfort and quality of life remain paramount.
- Deliver nursing care rooted in proven principles and techniques to ease pain and bring solace to patients.
- Educate and empower patients and families, encouraging their active participation in personalized care plans.
- Share the responsibility of call duties, demonstrating commitment to being available when it matters most.
Requirements
- Hold a current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
- Computer proficiency in typing and various programs, including EMR background.
- Possess and maintain current CPR certification if required by state.
