About the position
The Home Health RN Case Manager at Ohio Living is responsible for delivering nursing care directly to patients in their homes, fostering strong relationships with patients and their families. This role emphasizes independence, compassion, and adaptability in various home settings, while also providing ongoing assessment and care management. The position offers a balance of daytime hours and the opportunity to work with a supportive team, utilizing technology for patient records and care planning.
Responsibilities
- Provide ongoing assessment and nursing care to patients in their homes.
- Develop care plans and provide treatment based on patient needs.
- Work closely with patients and their families to ensure quality care.
- Maintain accurate patient records using mobile devices.
Requirements
- Current Ohio nursing license.
- One year of acute care or medical-surgical experience.
- Experience in a Medicare-certified home health agency preferred.
- Knowledge of EMR systems preferred.
- Valid driver's license, insurance, and acceptable driving record.
