About the position

The Registered Nurse Case Manager - Field Supervisor position at Prime Home Health involves overseeing the delivery of nursing care to patients in their homes. This role is crucial in ensuring high-quality care for pediatric and adult patients with complex medical needs, including those with feeding tubes, tracheostomies, and ventilators. The RN will manage clinical staff, conduct patient assessments, and coordinate care plans while providing education to patients and their families.

Responsibilities

• Submit OASIS forms for all patients on time
• Supervise and manage the delivery of nursing care to clients
• Review patient's clinical needs, updating diagnosis, medications, and procedures
• Conduct start of care visits at patients' homes
• Coordinate clinical notes and plans of care
• Review and update plans of care during home visits
• Train new field team members
• Attend periodic in-office meetings
• Counsel and supervise clinical staff
• Educate patients and caregivers on disease processes, medications, and treatment plans

Requirements

• Registered Nurse (Bachelor or Associate Degree) with at least 2 years of clinical experience
• Valid nursing license, CPR license, and driver's license
• Current PPD or chest x-ray required
• Strong management skills with the ability to supervise field staff
• Highly organized and ability to work independently
• Computer literate with the ability to learn new applications

Nice-to-haves
