About the position
HealthSource of Ohio is seeking a Registered Nurse (RN) to join our team. As a not-for-profit community health center in Southwestern Ohio, we are dedicated to providing quality, patient-centered primary and preventative care for all. The RN will play a crucial role in assisting patients through the healthcare system, acting as a patient advocate and navigator. This position involves coordinating continuity of patient care, educating patients and families, and participating in quality improvement initiatives. The ideal candidate will have a well-rounded clinical background, including experience in outpatient settings, and will be committed to treating every patient with respect and dignity.
Responsibilities
- Assists patients through the healthcare system by acting as a patient advocate and navigator.
- Coordinates continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
- Educates patient and family on ways to reduce ER visits in the future.
- Participates in Patient-Centered Medical Home team meetings and quality improvement initiatives.
- Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
Requirements
- Current RN Licensure in the state of Ohio
- Associates degree required; BA in nursing preferred
- 2 to 3 years clinical practice experience
- Well-rounded clinical experiences including work in outpatient settings
- Experience with data analysis and health care operations.
